How to Enroll

All new patients must:

1. Complete an Enrollment Application
2. Bring the required documentation in person to one of our clinic locations

Cost of Care

There is no cost for patient appointments. However, a suggested donation of $20 per visit helps support care for the next patient who walks through our doors.
